Liability Waiver and Release Agreement

1. Acknowledgment of Activities: I, the undersigned, acknowledge that I am voluntarily participating, and/or allowing my child/ward to participate, in camping, hiking, fishing, and traveling trips organized by Fathers Shaping Futures (“the Organization”) as part of its outreach and community engagement efforts. I understand that these trips are provided free of charge and are not commercial services.
2. Assumption of Risk: I am aware that participating in these activities involves inherent risks, including but not limited to, physical injury, illness, or property damage. I voluntarily assume all risks associated with my participation and/or my child/ward’s participation in these activities, whether known or unknown.
3. Release of Liability: In consideration of being allowed to participate in these activities, I hereby release, waive, and discharge Fathers Shaping Futures, its officers, directors, employees, volunteers, and agents from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or my child/ward, or to any property belonging to me or my child/ward, whether caused by the negligence of the Organization or otherwise, while participating in these activities or while on the premises where the activities are conducted.
4. Indemnification: I agree to indemnify and hold harmless Fathers Shaping Futures, its officers, directors, employees, volunteers, and agents from any and all claims, actions, suits, procedures, costs, expenses, damages, and liabilities, including attorney’s fees, arising out of or related to my participation or my child/ward’s participation in these activities.
5. Medical Treatment: I understand that Fathers Shaping Futures does not assume responsibility for providing medical care during these activities and that I will be responsible for any medical costs incurred because of my participation and/or my child/ward’s participation. I hereby consent to receive, and/or to allow my child/ward to receive, medical treatment that may be deemed advisable in the event of injury, accident, or illness during these activities.
6. Parental Authorization: As the parent or legal guardian of the minor participant, I hereby authorize my child/ward to participate in the camping, hiking, fishing, and traveling trips organized by Fathers Shaping Futures. I understand the nature of these activities and the risks involved. I have discussed these risks with my child/ward, and they understand and accept them.
7. Understanding and Acceptance: I have read this waiver of liability, fully understood its terms, and understood that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing the agreement freely and voluntarily and intend with my signature to have a complete and unconditional release of all liability to the greatest extent allowed by law.
